The program\u2019s goals include improving psychiatric stability, reducing the need for hospitalizations, reducing the stigmatizing effects of mental illness and promoting independence and community integration.<\/em><\/p>\n\n\n\n<p>Unified Community Services\u2019 Community Support Program is certified by the State of Wisconsin. Treatment is provided by an interdisciplinary team of professionals, such as psychiatrist, nurse practitioner, psychotherapist, social worker, and registered nurse.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Key Program Components<\/h4>\n\n\n\n<ul><li>Crisis intervention <\/li><li>Psychotherapy<\/li><li>Medication management<\/li><li>Psychiatric\/psychological services<\/li><li>Employment-related services<\/li><li>Social\/recreational skill training <\/li><li>Activities of daily living services<\/li><li>Assistance with: <ul><li>Applying for entitlements and vocational training<\/li><li>Obtaining physical\/dental health care <\/li><li>Obtaining legal services <\/li><li>Obtaining transportation services <\/li><li>Finding affordable housing <\/li><\/ul><\/li><li>Case management, including: <ul><li>Service coordination <\/li><li>Monitoring symptom status <\/li><li>Supportive psychotherapy and education <\/li><li>Advocacy<\/li><li>Support and education for family, friends, and community members<\/li><\/ul><\/li><\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Cost <\/h4>\n\n\n\n<p>Consumers are not billed for Community Support Program services.
