Unified Community Services (UCS) in Lancaster and Dodgeville has a full or half-time position to provide case management for persons diagnosed with substance use disorder, under the State Opioid Response (SOR) grant. Position will work collaboratively with prescribers and substance abuse counselors to improve treatment outcomes by providing services to include assisting clients with housing, employment, health insurance, transportation and other case management services, and grant-related data collection. Requires a bachelors degree in a human-services or related field, valid driver’s license and own transportation.
