Confidentiality

We will obtain your written authorization before using or disclosing your protected health information, except in some limited situations that are permitted or required by law. More information in Public Records Notice Section at the end of this page.

Cost

In many cases, health insurance, Medicaid or Medicare will cover some of or the entire bill. We bill insurance companies directly. If you have insurance for which Unified Community Services is not a provider, you will be referred to your insurance company to determine which providers are part of their network. There is a discounted/sliding fee schedule available* based on family size and income. No one will be denied access to services due to inability to pay.
*Fee for Intoxicated Driver Assessment may not be reduced or waived.

Unified Community Services has been legally established under Wisconsin Statutes 51.42/.437 and County ordinances.

Ben Biddick, Human Resources Generalist, has been designated as the person responsible for the “public records” held by Unified Community Services.   

The public may obtain information and access to records in Unified Community Services’ custody, make requests for records, or obtain copies of records and the costs thereof by contacting Ben Biddick weekdays between 8:00 a.m. and 4:30 p.m.

Responses to requests will be made per the requirements of Sec. 19.35(4)(a), Wisconsin Statutes. 

Fees for photocopying documents are .25 per page.  Additional fees may be charged to reflect the actual costs of other types of reproduction, location of a document where costs exceed $50.00, and for mailing or shipping expenses.  Prepayment may be required for any requests for reproduction of documents where costs exceed $5.00.
