We will obtain your written authorization before using or disclosing your protected health information, except in some limited situations that are permitted or required by law.

Cost
In many cases, health insurance, Medicaid or Medicare will cover some of or the entire bill. We bill insurance companies directly. If you have insurance for which Unified Community Services is not a provider, you will be referred to your insurance company to determine which providers are part of their network. There is a discounted/sliding fee schedule available* based on family size and income. No one will be denied access to services due to inability to pay.
*Fee for Intoxicated Driver Assessment may not be reduced or waived.
